What DiveSlip is

DiveSlip provides a telehealth medical evaluation for fitness to participate in recreational scuba diving and freediving. The evaluation consists of your completed diver medical questionnaire, a review of relevant medical records, and a live video visit with a physician licensed in your state. If the physician determines clearance is appropriate, you receive a signed Physician's Evaluation document. The determination is binary, approved or not approved, consistent with training agency standards.

What DiveSlip is not

DiveSlip is not emergency care, not primary care, and not a substitute for your own physician. The evaluation addresses dive fitness only. If you have urgent symptoms, call 911 or seek emergency care. Completing an intake does not create an ongoing treatment relationship beyond the evaluation itself.

Eligibility

You must be 18 or older, physically located in a state where our physician holds an active license at the time of the visit, and completing the evaluation for yourself. Minors need an in-person evaluation with a pediatrician or dive medicine physician.

Price and refunds

The evaluation is $91, charged once at booking. If the physician cannot safely clear you by video and refers you to in-person care instead, you receive a 100% refund. If you miss your visit, we will offer to reschedule you once at no charge. Refund requests: support@sickslip.co.

Honesty matters

The physician's evaluation relies on the accuracy of your answers and records. Providing false or incomplete information can make your clearance invalid and can put your life at risk underwater. We decline service where answers appear designed to obtain a clearance rather than an evaluation.

Telehealth consent

By booking you consent to receive a medical evaluation by synchronous video. Telehealth has limitations. The physician may determine during the visit that an in-person evaluation is required, in which case the refund policy above applies.

The document

Your signed evaluation carries the physician's name, NPI, state license number, and a verification code. Whether a specific dive shop, agency, or operator accepts any physician's evaluation is their decision. Clearance reflects your health on the evaluation date; training agencies generally treat it as current for 12 months.
