DiveSlip is an online physician evaluation for the PADI / RSTC diver medical. One board-certified physician reads your records, sees you on a short video visit, and signs your scuba diver medical form under his own name. It is a service of SickSlip P.A., the physician practice behind SickSlip.
Your doctor will not sign it. We understand why.
Most primary care physicians see a scuba diving medical form once a decade, and many decline to sign it out of unfamiliarity or liability concern. Dive medicine is a niche. Divers with manageable medical histories were being turned away, or left waiting weeks before a booked trip.
DiveSlip exists so that a diver with a yes answer on the questionnaire can get a real evaluation from a physician who knows the form and knows the water, without hunting for a dive medicine clinic in their city.
Dr. Adam Z. Kawalek, MD
Dr. Kawalek is a board-certified internal medicine physician with more than 15 years of hospital and emergency medicine, including Cedars-Sinai and Johns Hopkins. He is a PADI certified diver with dives logged in Belize, Hawaii and Bali, and he holds active medical licenses in 29 states. Every DiveSlip form is signed by him personally, under his own NPI and state license.

No scuba medical clearance is ever issued from a questionnaire alone. The physician reads what you send before you meet, then sees you on video.
Every form carries Dr. Kawalek's name, NPI and state license, and a verification code your dive shop can check at diveslip.co/verify.
If you cannot be safely cleared online, you get a referral to in-person dive medicine and a full refund, automatically.
