What is the PADI / RSTC diver medical form?

It is the standard Diver Medical Participant Questionnaire used by PADI, SSI, NAUI and most training agencies worldwide. If you answer yes to any screening question, the dive shop cannot train you until a physician completes and signs the Physician’s Evaluation Form. That signed form is what DiveSlip provides.
