Scuba medical clearance, signed online, in four steps.
Answer the diver medical questionnaire, meet the physician on a short video visit, and get your signed scuba diver medical form the same day. $91 flat. A full refund if we refer you instead.
Answer the diver medical questionnaire
The same nine screening questions that are on the PADI / RSTC Diver Medical form your dive shop uses, then a short safety screen. It takes about three minutes.
A few histories need an in-person evaluation with testing that video cannot replace: heart disease, a collapsed lung, lung surgery in the past year, seizures, insulin-treated diabetes, and pregnancy. If one of those applies to you, we tell you before you pay, and nothing is charged.
Book your video visit
Pick a time on the physician's calendar. Visits run most days of the week, and you see the real open times, not a request form.
If your dive shop gave you a Diver Medical form, upload it, signed on the participant page. Add any records that relate to your yes answers: recent clinic notes, spirometry if you have asthma, a cardiology summary if relevant, your medication list. Clear phone photos are fine. No form yet? Book now and send it later.
You pay $91 at booking. That is the whole price.
Meet the doctor on video
A live visit of about 15 minutes with Dr. Adam Z. Kawalek, MD, a board-certified physician licensed in your state and a PADI certified diver. He reads your records before you meet, so the visit is a conversation about your history and your diving, not paperwork.
Your signed form arrives the same day
When the physician approves your clearance, an email arrives the same day with your signed DiveSlip diver evaluation letter, plus your dive shop's own form completed and signed. Both carry his name, NPI and state license.
The letter carries a verification code. Your dive shop can check it in seconds at diveslip.co/verify or by calling (855) 346-3483.
Good to know
You get a referral to in-person dive medicine and every dollar back, automatically, without asking. There are no conditional clearances on this form; the physician either signs it or refers you.
Divers 18 and over, booking from one of the 29 states where Dr. Kawalek holds a license. You book from the state you will physically be in during the video visit, not the country where you plan to dive.
Most divers finish the whole process within one to two days of booking. The signed form is emailed the same day as the visit.
Training agencies generally treat a physician clearance as valid for 12 months. Keep a copy with your certification card.
