Telehealth Consent
What you are consenting to when you book a DiveSlip evaluation.

1. Nature and scope of the service
DiveSlip provides a telehealth medical evaluation of fitness to participate in recreational scuba diving and freediving. The evaluation consists of your completed diver medical questionnaire, a review of relevant medical records you provide, and a scheduled live video visit of about 15 minutes with a physician licensed in your state. When clearance is medically appropriate, the physician issues a signed Physician’s Evaluation. The determination is binary, approved or not approved, consistent with training agency standards.
The service:
- Does not provide diagnosis or treatment
- Does not prescribe medications
- Does not replace primary care or create an ongoing treatment relationship beyond the evaluation
- Does not provide emergency services. If you are experiencing a medical emergency, call 911 or seek immediate in-person care
Documentation integrity. The evaluation reflects the physician’s independent judgment based on your answers, your records, and the video visit. No clearance is issued from a questionnaire alone, and no clearance is issued that your information does not support. Providing false or incomplete information can make your clearance invalid and can put your life at risk underwater.
2. Live video telehealth
Your evaluation happens by synchronous video:
- You complete the intake and book a visit time.
- You meet the physician on a live video visit at the scheduled time.
- The physician may approve your clearance, ask for additional records, or determine that an in-person evaluation is required.
- There is no guarantee of approval. If the physician refers you to in-person care instead of clearing you, you receive a 100% refund.
3. Limitations and risks of telehealth
- There is no hands-on physical examination. The physician cannot listen to your heart or lungs with a stethoscope or perform in-person testing.
- Some histories require in-person dive medicine evaluation and testing that video cannot replace. In those cases the physician refers rather than clears, and you are refunded.
- The evaluation relies on the accuracy and completeness of the information and records you provide.
- Technology disruptions can delay or interrupt a visit. If a visit cannot be completed, we reschedule you at no charge.
- Electronic communications carry a risk of unauthorized access despite reasonable safeguards. Our safeguards are described in the HIPAA Notice.
4. Withdrawal of consent
You may withdraw consent and discontinue at any time before your evaluation is issued by emailing support@diveslip.co. Booking a visit constitutes consent to synchronous telehealth evaluation for dive fitness. Withdrawal after your signed evaluation is issued does not invalidate the document already issued. Records are retained per the Privacy Policy and applicable medical-records law.
5. Communications
By using DiveSlip you consent to receive transactional email: booking confirmation, visit reminders, your join link, and delivery of your signed evaluation. Email is required to use the service.
6. Acknowledgment
By booking, you confirm that you are 18 or older, that the evaluation is for yourself, that you are physically located in the state you selected at the time of the visit, and that you have read, understood, and consent to the above.
