HIPAA Notice of Privacy Practices
How medical information about you may be used and disclosed, and how you can access it. This Notice applies to protected health information for which SickSlip P.A. is considered a HIPAA-covered entity under applicable law.

1. Our legal duty
We are required by law to:
- Maintain the privacy of your protected health information (PHI)
- Provide you with this Notice of our legal duties and privacy practices
- Abide by the terms of the Notice currently in effect
- Notify you following a breach of unsecured PHI as required by law
2. How we may use and disclose your PHI
Treatment. We use and disclose PHI to conduct your dive medical evaluation: reviewing your diver medical questionnaire answers, reviewing records you provide, conducting your video visit, and issuing your signed Physician’s Evaluation.
Payment. DiveSlip operates on a cash-pay basis and does not bill insurance. Our payment processor handles your card and gives us only payment status.
Health care operations. We may use and disclose PHI for quality assessment, compliance monitoring, and fraud detection.
Verification. Your signed evaluation carries a verification code. The public verification page confirms the document is genuine and current; it does not display your health information.
As required by law. We may disclose PHI when required by federal or state law.
3. Your rights regarding PHI
Right to access. You may request a copy of PHI maintained in a designated record set. Email support@diveslip.co.
Right to amend. If you believe PHI in your record is incorrect or incomplete, you may request an amendment.
Right to confidential communications. You may request that we communicate with you through alternative means or at an alternative address.
Right to an accounting of disclosures. You may request a list of certain disclosures we have made of your PHI.
Right to receive a copy of this Notice. You may request a paper copy at any time.

This Notice is effective for the DiveSlip service line as of July 24, 2026.
